Selena Gomez arrived at the 2026 Emmy Awards looking every bit the Hollywood star. The 34-year-old singer and actress stepped onto the red carpet in a shimmering golden Louis Vuitton gown, her hair styled in large, glamorous curls and her makeup soft and peachy. But instead of focusing solely on her dazzling look, some viewers became fixated on something else entirely — her face.
Within hours, social media was flooded with speculation. Some commenters wondered whether Gomez had recently undergone Botox or fillers. Others questioned if she had used weight-loss medication. The conversation quickly turned into a full-blown debate about cosmetic procedures — and once again, Gomez found herself at the center of it.
“How Much Botox Was Put on Her?”
The reaction online was immediate and intense. “How much Botox was put on her,” one person wrote on X. Another added: “The Botox really ruined her face.” The comments echoed a familiar pattern — a pattern Gomez has been forced to confront for years.
But according to one plastic surgeon, the truth may be far less dramatic than the internet assumed.
A Plastic Surgeon Weighs In
Dr. Omar Tillo, a plastic surgeon and medical director at CREO Clinic, told The Blast that he would be “very cautious” about attributing Gomez’s appearance to any specific treatment based on photographs alone. Instead, he pointed to what he described as an apparent difference in the symmetry around her eyes.
“What people seem to be responding to most strongly in these photographs is the asymmetry around the eyes,” Tillo explained. “One eye appears slightly more closed than the other, and because the eyes are such an important reference point when we recognize a face, even quite a small difference can make somebody look surprisingly different.”
He acknowledged that Botox is one possible explanation for that kind of appearance — but emphasized it is far from the only one. According to Tillo, Botox temporarily reduces activity in targeted muscles. When used around the forehead or between the eyebrows, dose, placement, and distribution matter because several muscles help control eyebrow and upper-eyelid positioning. If those muscles are affected unevenly, a person can temporarily see a difference in brow height or how open one eye appears compared with the other. In uncommon cases, Botox can also cause temporary eyelid or eyebrow drooping known as ptosis, creating noticeable asymmetry.
Timing matters, too. The effects of Botox generally develop over several days and become more established during the following week or two. That means someone who recently received treatment could still be seeing the results settle.

But Tillo stressed that none of this means Gomez actually had Botox before the Emmys.
What About Fillers?
Some viewers also focused on Gomez’s cheeks, claiming they looked fuller and wondering whether dermal filler was responsible. Tillo said that was impossible to determine from photographs as well.
“Facial expression, camera angle and existing natural asymmetry can produce a very similar effect,” he said. He added that makeup, lighting, and styling can also dramatically change how facial proportions appear on camera.
Gomez Has Addressed the Rumors Before
This isn’t the first time Gomez has faced speculation about her appearance. In 2023, she directly acknowledged having Botox while responding to a fan on social media: “Hahahaha I’ve had Botox bb girl.”
The following year, she once again found herself addressing cosmetic-procedure speculation after commenting on a video from plastic surgeon assistant Marissa Barrionuevo. Barrionuevo said she had no idea what procedures Gomez may have had and pointed to the singer’s history of medical issues as one possible reason people shouldn’t speculate about her appearance. Gomez eventually responded herself.
“I have Botox. That’s it. Leave me alone,” she wrote.
The Medical Reality Behind the Scrutiny
Gomez has been open about being diagnosed with lupus, an autoimmune disease, in 2013. She underwent a kidney transplant in 2017 at age 24. Her treatment has included steroid medications, which are known to cause side effects including water retention, weight gain, and a condition commonly called “moon face” — a round, puffy appearance caused by the medication’s impact on fat distribution and fluid retention.
As Dr. Sermed Mezher explained in a previous video: “They can cause side effects, like water retention, weight gain and something called ‘moon face,’ where we become puffy and more rounded.” He urged viewers: “Let’s set our gaze more than skin deep.”
Gomez herself has spoken about how her medical history has affected her appearance. “I’ve dealt with a lot of weight issues in my life, and that’s something I’m very sensitive to,” she told Allure. “Now I understand, that’s stemming from that one time when I was going through some medical stuff and I had gained weight.”
“Anything More Specific Would Simply Be Speculation”
Ultimately, Tillo emphasized that there is a major difference between analyzing a photograph and examining a patient.
“Without examining somebody clinically and knowing what treatment they have actually received, anything more specific would simply be speculation,” he said.
Gomez has not said she recently received Botox or any other cosmetic treatment.
The Bigger Conversation
The debate surrounding Gomez’s Emmys appearance raises a question that extends far beyond one red carpet: Why do we feel entitled to dissect women’s faces in the first place? And when a person is living with a chronic illness that can visibly change their appearance, what does it say about us that our first instinct is to speculate rather than to empathize?
For Gomez, the answer has been consistent: she’s had Botox. That’s it. And she’d like to be left alone.
